Here is the regular card #5 from "Sailor Moon S : Jumbo Carddass Card : Part 1" released by Bandai in 1994.

CARD CONDITION : The card is in fair and damaged condition. There are a lot of some dents along edges and on the corners. There are a lot of scratch marks on the front and back. The corners are damaged and there are heavy dent and crease marks on the front and back. There is a lot of wear from handling.

Made in Japan.
